Apply for the AHT credential

Step 1

  • I have read and checked eligibility and requirements.
  • Complete five core courses. Use the course schedule website page to plan ahead.

Step 2

Forms and references referred to below can be accessed and downloaded from the 'Forms and references' page of the website.

  1. Download the Reference HTCC Mentorship Handbook, The Hand Therapist Peer Mentoring Manual, 2nd Edition
  2. Complete the Form AHT01–Mentorship Contract.
  3. Complete the Form AHT01A–HTCC Self Assessment Tool.
  4. Upload the five core course attainment certificates to your CPD Log. You can find this in your membership portal.
  5. Prepare to upload Forms AHT01 and AHT01A and pay the application fee.
  6. Commence an online application via the 'Apply Now' page.
  7. Await formal advice on mentorship approval before commencing mentorship.

Step 3

When you receive your mentorship approval, download Form AHT02–Mentorship Log. This Mentorship Log will be required for recording activity throughout the minimum twelve-month mentorship program.

Step 4

  • Complete two elective courses.
  • Upload attainment certificates for these courses to the CPD log in your membership portal.

 

Step 5

Submit your final application for approval.

1. Prepare these forms for your online application:

  • AHT02–Mentorship Log
  • AHT03–Mentor Verification Form
  • AHT05–Employment Verification Form
  • AHT06–Statutory Declaration Form for Sole Traders.

2. Go to the ahpra.gov.au and then select 'look up a practitioner'. Please enter your Ahpra number or search for your name. A page will appear with your registration details. Print this for upload.   

2. Upload all of the above documentation.

4. YOU MUST email the office via Contact us to advise that you have submitted all the required documentation and are now ready for the Credentialing Council to review your application.

5. Wait for final approval by the Credentialing Council and the AHTA Board. This process can take up to three months, depending on the date of the final application and the scheduling of the meetings of the Credentialing Council and Board.

Step 6

Add your details to the AHT Directory on the AHTA website.
