Ensure you have read Step 1 Join the AHTA and Step 2 Application Information prior to completing this form.

Please set your own user name and password when requested on this application. This will be your login details for the AHTA website.

Your complete application should include:

  • Completed  Online Application Form (click to view)
  • Letter of recommendation
  • Copies of qualifications
  • National Registration
  • Employment Verification Form (Full Member only)
  • Professional Development Education form (Full Member only)

PLEASE NOTE: Copies of qualifications and other suporting documentation should be submitted directly to the AHTA Office via either email info@ahta.com.au, fax +61 8 9049 1677, or mailed to Membership Secretary at PO Box 84, Southern Cross, WA 6426 Australia.

Application (Administration) Fees*:
New Full Member $77.00
Upgrade to Full Member $77.00
Associate $44.00

*All applicants, and those applying to upgrade their membership from Associate to Full Member, will be required to pay a non-refundable Application Fee. This is an administration fee and is in addition to subsequent membership fees. Once applications are approved by the AHTA Management Committee, new members are invoiced for membership fees to the end of the current calendar year.
